• Simple transition process. We offer a sim-
ple four-step process to transition to ACHC at any time in your accreditation cycle, including:
° Registration—Submit your transition registration form and $500 deposit
° Review—Transition documents are reviewed to ensure regulatory compliance and confirm good standing of current accreditation.
° Education—An ACHC Clinical Educa- tor will assist with P&P, clinical questions, and survey preparation.
° Approval—Upon successful completion of the clinical review and receipt of contract, the transition will be approved. ACHC approval documents will be issued thereafter.
When you transition to ACHC, you’ll receive a free digital copy of the ACHC Accreditation Guide to Success workbook ($399 value). The workbook was specifically designed to help
and procedures. It will also provide you with audit tools, accreditation preparation, and best practices for your survey.
